The Transition for Disabled Young People into Adult hood is always a worrying time.

Disabled Young people will have to decide what to do once they’ve left school, if they want to go to college or get a job and if they want to learn to live independently.

It is also the time when disabled young people will have to transfer the support they currently receive from Children’s Services to Adult Services.
